The ashes from a growth Chuck Norris had removed (more powerful then angle dust cut with jesus' zombie ash).

It supposedly contained the ashes of his parents. Undertaker was influenced by it because it was the only thing he ever felt guilt about. Then he found out that Kane burned the house down, and not him...

Undertaker's urn lost a lot of power after that, as he overcame the power of the urn, to turn on Bearer and continue the Kane/Taker feud.

The urn returned for a short while, along with Bearer, explaining that Undertaker was the older brother and he had responsibility over Kane's actions... bringing back the guilt and the power of the urn. Undertaker once again broke out of the urn's influence when he buried Bearer in concrete and killed him.

It was empty. At one point, Kama, aka the Godfather, had it melted down and turned into a chain. Also, once at SummerSlam, Paul Bearer opened it in the middle of the ring and there was a huge spotlight coming out of it. It was during the Undertaker's entrance in the infamous Undertaker vs Undertaker match.
