Lol, yeah it does look painful when he does that. However I bet his opponents hurt even more so with their face/head being planted to the mat. I'd say it's more of his ribs/sides that must get the bump more than his back would as he kinda does his move sideways.

Of course. Whenever Boreton does the RKO, he hurts himself more than he does to his opponent.
