The Undertaker is waiting under the ring. There is a TV under there, so he knows when it happens (though the bell sound also tells him). He then comes out to the commentators side (most people look at the ring or the entrance area, so he generally goes unnoticed by flashing cameras) and slowly slides under the ring ropes and stands in the ring. He has to slide under the ring ropes slowly so as the ring ropes don't move, which would give away where he came from.

He actually hides under the ring. The lights are out for more than what they show on tv. Like lets say when watching him appear on tv the lights are out for 5.7 seconds, in actuality the lights are out 20-30 seconds enough time for him to make it in if he come from the back.

I went to only one show when he done it back in 2007 and it was very clear EVEN 6 rows back watching him crawl from under the ring and slide under the rope and stand straight up.When you see the camera flashes it's like a damn light and very easy to see live.Same thing with Edge when Taker pulled him down and thru the ring.
